What is the Community Reintegration Program?
The Community Reintegration Program is designed to assist people currently living in nursing homes or other long-term care facilities to live independently in the community.
Persons with disabilities living in nursing homes often face barriers to leaving such as:
- isolation from the community
- lack of resources to pay rent and the security deposit
- inability to purchase other necessary items to create a home
- lack of family support
- lack of information regarding available options; etc.
IVCIL’s Community Reintegration Program helps people with disabilities to overcome these barriers and assist them in moving back into their community of choice.
WHO MAY QUALIFY FOR THE PROGRAM?
A person with a long-term disability
- who currently resides in a nursing home
- who is under the age of 60 except in certain cases
- who has less than $17,500 in assets (a car and house do not count)
- who is willing and able to take charge of his or her life
- whose doctor must agree to discharge
WHAT CAN IVCIL DO FOR YOU?
- identify and secure community services and resources
- locate and assist with applying for appropriate public or private housing in the community
- provide financial assistance with:
- down payment of rent, utility deposits, and hook-up charges,
- furniture, appliances, curtains, dishes, linens, clothing, initial cleaning supplies,
- an initial stock of food staples, and other necessities that are approved
- the purchase of equipment or assistive devices, although funding may come from other sources
- the cost of essential home remodeling for accessibility, although funding may come from other sources.
- work with nursing home staff to ensure a smooth transition
- after the move, IVCIL will continue to visit and provide advice
